This award recognizes frontline caregivers from any department for their contributions that lead to improved quality of life for the residents and create a better work environment for the adult care / assisted living community’s staff. This includes, but is not limited to, direct care staff from nursing (does not include licensed nurses), housekeeping, dietary, activities / recreation, laundry, and maintenance departments.

All nominees must be employed by a NYSCAL member community in good standing at the time of nominations. The nominee:

  • Contributes to the positive well-being of the residents and the morale in the adult care / assisted living community
  • Demonstrates outstanding person-centered care and achieves a heightened quality of life for adult care / assisted living residents
  • Shows initiative and performs above and beyond the call of duty. Provides comfort to the residents, family members, and volunteers
  • Empowers the residents to achieve individual autonomy, dignity, and quality of life in their daily living
  • Has a positive, outgoing attitude
  • Displays leadership, integrity, and a high standard of ethical behavior • Shows respect for residents, co-workers, family members, and volunteers at all times

Please Provide the Following Information about Your Nominee:

  • Each nomination should be accompanied by an essay or letter from the administrator or supervisor outlining why the nominee should receive the Noble Caregiver in Assisted Living Award. It should address how the nominee meets the award criteria and how the nominee incorporates the criteria into the daily routines. Use as many examples as possible. In the essay, please refer to the person as “the nominee”, and do not identify the adult care / assisted living community or location. Please limit the essay to a total of 250 words and attach this page as the cover sheet. Do not submit pictures with your nomination form and related materials.
